• Your full name, mailing address and daytime phone number.
• A packing list (typewritten or printed preferred) including all contents of the shipped equipment (Power cords, footswitches, other accessories, etc.)
• A Bill of Sale for any warranty repairs.
• A description of the equipment failure (e.g.- Does not power up, crackling and distorted sound from channel 2, etc.)
• Any special instructions (e.g.- written request for estimate, repair as soon as possible, etc.)
